The Hospice at Home team work 24 hours a day, 7 days a week providing care to individuals and their families to assist hospital discharge, prevent unnecessary hospital admissions, provide complex symptom control, and support patients in the final stages of their illness. Our team also manages OneCall advice line, which provides support and specialist palliative advice to community healthcare professionals, individuals and family members. Their specialist care allowed Doris to pass away peacefully in her family's hands.
